Steps Involving in Creating GOOGLE FORMS

Step 1: Sign in the google drive.

Step 2: Click On new.

Step 3: Go to More --> Google Form

Step 4: Untitled Form is open Edit it As per Your Requirement.

I m going to create Registration Form Which Include

Name

Email Id

Phone Number

Branch (Radio Buttons -- Multiple Choice) It is option Type

Step 5:Name The form like Registration form and

Replace The Untitled Question With "Name"

Step 6:In right It gives option for Which type of text will be select Short Answer.

Step 7:In Right Corner Their is + symbol Click to add new Question.

Step 8 : Repeat Step 5 , 6 and 7 as per your requirement.
